To ensure you have a great time here at Virtual Reality Breckenridge - 

I am acknowledging the following for myself or Child/Minor/Dependent. 

I (or my Child/Dependent/Minor) wish to use VR Arcade Breckenridge’s headsets HTC Vive Pro and I understand that the use of the Virtual Reality headset involves certain risks which can include:

1. Virtual Reality is a new concept to the entertainment world.  Due to the unpredictable nature of the human response to virtual reality, which can include dizziness, nausea, seizures, fear of heights, bumping into objects, since of vertigo, plus other bodily responses - sweating and heavy breathing from fast paced games etc. 
2. If you ever feel any discomfort during a game - Please STOP immediately.  
3. If I (or my Child/Minor/Dependent) cannot follow the VR Attendant’s instructions, when putting on head gear you will not be allowed to play and will be compensated accordingly.
4. I will be respectful of the equipment and will be held responsible for any fowl play - Throwing the controllers or ripping off headsets.  Normal wear and tear not included. *Assume all of the physical, psychological, and financial risks associated with the use of the VR equipment.
5. I am using the equipment voluntarily.
6. I may not stay in the confined area and may sustain injury.

BY USING ONE OF THE HTC VIVE PRO HEADSETS, YOU ARE INDICATION YOUR ACCEPTANCE OF THE TERMS AND CONDITIONS OF THIS AGREEMENT.

By signing, I acknowledge that I have read and understood all the terms of this release form and that I am voluntarily giving up substantial legal rights, including the right to sue VR Arcade Breckenridge or it’s employees.  

**[If the headset is being used by a Child/Minor/Dependent] I am the Parent or Legal Guardian of the Minor named below.  I have the legal right to consent and and by signing below, I hereby do consent to the terms and conditions of this Waiver.

I understand that I must sign this waiver before the minor I am giving permission for, may use any of the Virtual Reality Equipment.
