• Participation in a Rage Room involves inherent risks, including cuts, lacerations, bruises, eye injuries, flying debris, broken glass, physical exertion, serious injury, permanent disability, and death.

 • I voluntarily assume all risks associated with participation.

 • I release and hold harmless Panic Escape Room, its owners, employees, managers, contractors, landlords, and affiliates from liability arising from participation, including claims resulting from ordinary negligence to the fullest extent permitted by Illinois law.

 • I agree to wear all required protective equipment, including eye protection, face protection, gloves, and any additional equipment required by staff.

 • Closed-toe shoes are required.

 • I will follow all staff instructions and understand my participation may be terminated without refund for unsafe behavior.

 • I will not participate while under the influence of alcohol, drugs, or any impairing substance.

 • I understand Panic Escape Room is not responsible for lost, stolen, or damaged personal property.

 • I understand that only designated items may be destroyed and that I may be financially responsible for intentional damage to property not designated for destruction.

 • Participants must be at least 12 years old.

 • Participants ages 12–17 must have a parent or legal guardian sign this waiver and must be accompanied by a responsible adult age 18 or older who remains on-site throughout the experience.

 • I authorize emergency medical treatment if necessary and accept responsibility for any resulting costs.

 • I have read and understand this waiver and voluntarily agree to its terms.
