I hereby acknowledge that I shall be solely responsible for all damage resulting from my actions including but not limited to damage to the real estate were the activities occur, damage to the props, equipment, and all other property of Countdown Louisville.

I hereby acknowledge that Countdown Louisville uses live security cameras in escape rooms and lobby for operational and security purposes.

I hereby grant permission to Countdown Louisville to store my contact information, I understand it is forbidden to photograph or video game elements and that it is forbidden to copy or record any aspect of the game for any purpose.

I hereby acknowledge that I have read and understand this document and fully understand its content, I am aware that this document serves as a release of liability and is a contract between myself and Countdown Louisville, all information I enter is accurate, and I sign it of my own free will.  I hereby acknowledge receipt of a live escape game experience, on this date, from Countdown Productions, INC DBA Countdown Louisville, 1501 Envoy Circle, Louisville KY 40299.
