WAIVER, NON-DISCLOSURE AND RELEASE OF LIABILITY FORM

I hereby acknowledge that Big Escape Rooms LLC has described to me the risks associated with playing an escape game, has infomed me not to run in the room, not to move heavy furniture and not to step up onto anything in the room.  I have been told to be careful of slipping while in the room and I have been instructed on how to exit the room in case of emergency.  I have also been warned of low doorways, which add to the effect of the game, but must be treated with respect at all times to avoid injury.  Big Escape Rooms LLC will not be held responsible or liable for any injuries sustained by me while at Big Escape Rooms LLC.

I hereby acknowledge that Big Escape Rooms LLC has offered to keep my personal belongings in their control room while I play the game or allow me to take my belonings with me into the room.  Big Escape Rooms LLC will not be held responsible or liable for my personal belongings while at Big Escape Rooms LLC.

Big Escape Rooms LLC has informed me that some of the props in the rooms can be delicate or broken easily.  I have been asked to handle all of the props with care.  I hereby acknowledge that I will be held solely responsible and liable for damages caused by me to the property of Big Escape Rooms LLC as a result of my misconduct.

I hereby grant permission for Big Escape Roomss LLC to use photo and/or video content containing my likeness for professional purposes.

I hereby grant permission for Big Escape Rooms LLC to store my contact information for operational purposes only, unless I grant further permission as below.

I hereby grant permission for Big Escape Rooms LLC to store my mobile phone number and e-mail address for the purpose of SMS and direct e-mail marketing, although I will be able to opt out of these notifications at any time.

I understand that a large part of the business success of Big Escape Rooms LLC revolves around keeping the propietary information of the room designs a secret.  I agree to not disclose to anybody else, any specific details of the escape rooms that I participate in.  This includes, but is not limited to types of locks, types of puzzles, number of rooms or sections, the solutions to puzzles or riddles or the general layout of the rooms.    I hereby acknowledge that I will be held solely responsible and liable for damages caused by my disclosure of any sensetive information, as described above, to any other person or people. 

I agree that this agreement shall be governed exclusively by the laws of the State of Georgia.
