PUZZLEWORKS LLC RELEASE OF LIABILITY FORM

In consideration of being permitted to participate in PuzzleWorks, LLC,’s (PW) puzzle rooms and any and all current and future PW events or activities, collectively “Activities”, I, on behalf of myself, my heirs, my spouse, my family, my descendants, personal representatives, next of kin or other representatives, agree to indemnify, defend, and hold harmless PW, its members, managers, governors, employees, agents, affiliates, or subsidiaries, and all other persons or entities against whom I may bring or be able to bring a claim against (hereafter collectively "PW") from any and all claims, damages, losses, injuries and expenses arising out of or resulting from participating in the Activities. I further agree to release, acquit and covenant not to sue PW for any and all actions, causes of action, claims or damages, damages in law or remedies in equity of whatever kind, including the negligence of PW or of myself, I may have against PW arising out of my participation in the Activities. In short, I cannot sue PW and if I do, I cannot collect any money. My spouse, family, descendants, successor, heirs, next of kin, personal representative, and estate are bound by my promises to PW.

As liquidated damages, I hereby agree that if PW is forced to defend any action, lawsuit or litigation caused or started by myself, my personal representative, my spouse, my family, my heirs, my descendants, or my next of kin, I, or my personal representative, agree to pay PW the costs and attorney's fees to defend such action, lawsuit or litigation regardless of the outcome and if for any reason this release of liability were not upheld in court, to pay also the full judgment against PW.

I have adequate health, disability, and life insurance for myself, and acknowledge that PW does not provide accident or medical insurance for me.

I, of my own free will, for myself, my spouse, my family, my descendants, my heirs, personal representatives, other representatives, estate and next of kin, have read, understand and acknowledge the risks and disability for myself.

If the participant for whom this waiver is being filled out is a minor, I am the parent or guardian of the minor named below and have the legal authority to execute this release on his/her behalf. I have read the agreement and I understand its meaning. In the event of any future claim brought by or on behalf of him/her arising from or related to his/her participation in the Activities or other claims related to this release, I hereby agree to hold harmless, defend and indemnify PW from and against any such claim(s). I have read this document in its entirety and I am executing it willfully, with full knowledge of its contents and with an understanding of its consequences.
