Disclaimer: I accept full responsibility for my actions and will not hold ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC liable for harm or injury sustained during any activities within the facility. Further, I will accept the rules and judgments of the staff of ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC as final and binding.

By signing this waiver form, I release and forever discharge, ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC employees and other representatives from any and all damages and causes action either at law or in equity that I may have as a result of participation in, attendance at, and interaction at any event and incident within the property of ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC. Furthermore, I do hereby expressly stipulate, and agree to indemnify and hold forever harmless the ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC employees, and other representatives against loss from any and all present or future claims, demands, or actions in law or in equity that may hereafter be made or brought by me or my child, or by anyone else on their own behalf for damages or any other legal or equitable remedy on account of any injury, illness, physical condition, inconvenience, or loss sustained by me or my child during any events that occurs within the property of ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC.

I give permission to ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC, or anyone authorized by ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC, to take photographs of me or my child by use of a camera, video or other film equipment. These photographs may be reproduced and used for the purpose of ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC promotion programs or advertisements.

I acknowledge and agree that the photographs of me or my child taken by ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC shall be the exclusive property of ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC and can be reproduced without compensation or payment of any kind to me or any other person.

 For Parents/Guardians (if under 18): Precautions are taken for the safety and health of your child, but in the event of an accident or sickness, ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC, and its staff are hereby released from any liability, in the event that your child conducts him/herself in a manner that is disruptive, he/she will be dealt with appropriately, including the option of contacting the parents and/or sending the child home at the expense of the parents.

  In the event that your child requires special medication, x-ray or treatment, the parents/guardians will be notified immediately, however ESCAPE ARTIST KINGWOOD,LLC and/or ESCAPE ARTIST WOODLANDS,LLC and its staff will not hold any responsibility for any damage or impact that occurs to your child during the course of this incident.
