I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS ESCAPE ROOM EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fail.

*I agree to wear a face mask while onsite and to use provided hand sanitizer before & after my game. I am aware of, and agree to follow, local health unit, provincial and federal guidelines regarding COVID-19. 

*I answered "NO" to all questions on the Ontario COVID-19 screening tool posted on Trapdoor London Escape Room's front door.

*Current COVID-19 framework guidelines require collection of contact information for all patrons. 

1. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Trapdoor London Escape Rooms ("Trapdoor"), or the employees, representatives or agents of Trapdoor. I agree to pay for all damages to the facilities of Trapdoor caused by my negligent reckless or willful actions.

2. I have no physical or mental illness that precludes my participation in a safe manner for myself or others. I am not under the influence of drugs or alcohol which impairs my ability to maintain my safety or endangers others. I acknowledge that this Waiver will be used by the organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. I agree that all staff or authorized agents, may, in their sole discretion, determine it is unsafe for myself or others for participation to continue, and remove me from the premises by any lawful means.

3. I agree that Trapdoor has the right to any photos or any video/sound footage of me during this activity. The photos, video footage and sound materials may be used for any marketing purposes and/or posted on Trapdoor's social media accounts. I will not post trade secrets or game puzzles from inside any Trapdoor games on any social media.

4. I certify that I understand this activity has potential risks including but not limited to:

• Use of simple tools
• Potentially moving or lifting objects of not more than 10 lbs
• Being in a reasonably small space with up to 8 persons
• Potentially climbing up a step or navigating unusually shaped spaces
• Mental stress and anxiety
• Possibility of failure to escape the room in the allotted time
• Possibility of falling objects

5. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, heirs, next of kin, successors and assigns as follows: 1. Waiver, release and discharge from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released - for my personal injury, death, property damage, property theft or actions of any kind that may hereafter occur to me - the following entities or persons: the directions, officers, employees, volunteers and agents of any and all entities authorizing this activity; 2. Indemnify, hold harmless and promise not to sue the entities or persons mentioned in this paragraph from any and all liability or claims made as a result of participation in this activity.

This Wavier and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under the applicable law. I certify that I have read this document and I fully understand its content. I am aware that this is a release of liability and a contract and I sign it of my own free will.
